| Comprehensive In-Home Assessments |
| Comprehensive in-home assessments address activities of daily living, medical, social, emotional, nutritional, financial and safety concerns. Assessments are provided to determine both immediate and future goals necessary to enhance our clients' quality of life. We consider individual needs, and offer comprehensive service options that respond to those needs. |

| Individualized Care Plans |
| Once an assessment is completed, a comprehensive Care Plan is developed based on individual needs. Recommendations may include home care, environmental changes that enhance safety, and referrals to community services. |

| Coordination of Home Care |
| We arrange appropriate home care & provide oversight and monitoring of care givers to ensure they are meeting all of our clients' needs. Caregivers may provide assistance with daily grooming, bathing, showering and hygiene; light housekeeping; shopping; preparation of nutritious meals; assistance with walking and mobility; accompanying clients to social events and reducing isolation & loneliness. |

| Advocacy Services |
| We act as an advocate and liaison between older adults, families and community services including health care providers, elder law attorneys, social service agencies, long term care insurance companies and government entitlement programs including Medicaid and Medicare. |

| We provide information, education and support related to making decisions, problem solving and stress management when caring for older adults. |

| Medication Review and Management |
| We confirm appropriate medications with prescribing physicians and provide ongoing review of medications and accompanying side effects. |

| We respond to emergencies and crises with immediate support, visits and referrals. |

| When necessary we explore appropriate options for assisted living, Continuing Care Retirement Communities (CCRC) or skilled nursing facilities. We assist clients and families with admissions applications, interviews and moving. |

| We schedule appointments, arrange transportation and accompany clients to community appointments. |
